A PDF version of the US 1820 census form is shown here on Ancestry.com.  Each page of the form had room for 22 households - paper wasn't cheap back then.

Here's another view, reconstructed from the Ancestry.com version.  The names of the heads of each family are written horizontally in the first, wide column in the 22 bottom rows. The large blank rectangle at upper left is for location information to be entered by the census-taker.  I've reduced the number of rows made available for separate households.
